Waiver of Liability and Responsibility
 

I, ________________________, certify and acknowledge that I understand this form is a waiver of any legal right against The Paddle LLC and its facility and by signing this form:

That The Paddle LLC, has advised me prior to my commencement of participation in paddle boarding could result in physical injury or even accidental death.

That I will follow the instructions and rules listed below:

1. I am not under the influence of any alcohol or drugs and will not consume any alcohol or drugs for the entire duration of my rental.

2. I will wear the provided life jacket the entire time I am on the paddleboard or kayak.

3. I will not lend the paddleboard to anyone for any reason during my rental.

4. I will not go through the Montlake cut and will stay within Lake Union throughout the entire rental.

5. I will monitor my time and exercise exertion so I can make it back when my rental reservation ends or by closing time at 8:00 pm.

6. Your pet’s safety is your sole liability and responsibility.

That I, ________________________, freely and knowingly assume the risk in renting a paddleboard/kayak, and I hereby waive any right, claim, or cause of action against The Paddle LLC and release them from liability for any injury, cost, damage expense or claim, which I or anyone on my behalf might incur as a direct or indirect result of my participation of Paddleboarding or Kayaking.

 

That I, ________________________, will call The Paddle LLC immediately if there are any equipment issues or if there is an emergency, and in reading this, understand that if I need to be picked up by The Paddle LLC, there is a $50 retrieval fee.

 

That I, ________________________, understand that any damage to ay rental products during your trip is subject to a repair or replacement fee. Repair fee is $100, replacement fee is $500.

 

That I, ________________________, have read this Liability Waiver form, understand and agree with each of the foregoing points.
